When I called them for an explanation, they told me that it is a standard letter that they send so Royal Caribbean gives you a voucher. OP needs to get RC to track the payment and confirm whether they sent the insurance company the payment. If yes then Insurance co is liable. If no then blame lies with RC. If Insurance company refuse to pay and they were paid then possibly OP could go after RC under vicarious liability as Insurance company is providing service on their behalf.
